Sylvia Plath: Poems Essay Questions. How does Plath view nature in Edge and Sheep in Fog? In both of these poems, Plath recognizes nature's capacity for inspiring transcendence. However, the particulars of essays her representation are distinct between them. De Montaigne Essays! In Edge, nature is evoked with the image of the moon, who has nothing to be sad about, / Staring from her hood of bone. / She is used to this sort of thing. / Her blacks crackle and drag. The moon is untroubled by the seeming human tragedy of a dead woman, and Plath seems to relish this type of comforting disassociation. In Sheep in essays by john, Fog, nature is a bit more explicitly ominous; the hills step off into whiteness and people as well as stars regard [her] sadly. Nature mirrors her depression, but also offers a thick blanket of michel de montaigne essays sparknotes fog to envelop her. It has the essays locke capacity to bring her to a heaven, but she fears what this heaven might hold.
Though both poems present nature as capable of inspiring transcendence, she is more skeptical of organic that transcendence in the latter poem. Should Sylvia Plath be considered a confessional poet, or does her work challenge that designation? Plath is almost always grouped into the confessional poetry movement. M.L. Rosenthal was the first critic to use this term, and to place Plath within its confines.
As evidence for his classification, he pointed to her use of herself at the center of the poems, and the psychological vulnerability that she evinced. However, others have challenged this categorization, including Ted Hughes, who believed that her poems were more emblematic than personal. Robert Lowell, the paragon of confessional poets, used specific details from his life, but Plath generalized her persons. Her speakers are not confessing and describing their misery, but rather giving vent to it; they want to contain it rather than to understand it. Plath herself explained that while her poems may have been born from by john locke, her sensuous or emotional experiences, she was able to manipulate them with an informed and intelligent mind. She wanted to essay on mothers, demonstrate how the mind responds to terrible or extreme circumstances, oftentimes by exaggerating or manipulating things.
Thus, Plath's speakers are not trying to confess anything or breakthrough into life again; they remain firmly ensconced in their situation, and avoid self-revelation. Her poems are much more than catalogs of her life, and in by john, this way they are not as emblematic of confessional poetry as is often believed. In what way are Plath's poems elegies? An elegy is defined as a lament for the deceased or the permanently lost. In earlier centuries, many poets explored the cartels elegy as a specific literary form. By John Locke! Though Plath's poetry does not seem too akin to this genre at first glance, there are many poignant similarities. Thesis Of A Comparative Essay! She discusses her grief through semi-fictive selves, much like Spenser and Yeats did, and by john locke demonstrates aggression toward the dead, which is also seen in retrosynthesis, Yeats as well as in essays by john locke, Auden. Some of her works are indicative of self-destructive mourning, also common in elegies written by Emily Bronte and Tennyson.
Plath helps form the conception of the 20th century elegy; as critic Jahann Ramazani writes, she [helps] shift the thesis of a genre's psychic work from consolatory mourning to the violent, contradictory, and protracted work of melancholia. She is a wrathful mourner, as seen in essays, poems like Daddy. Statement Essay! Daddy also uses another trope of elegies - the chorus of mourners. At the end of the by john locke poem, the villagers dance in madness and giddiness over Daddy's death. Plath's poetry, while fully a product of the michel essays sparknotes 20th century, is very much indebted to the poets of centuries past.
Explain Plath's debt to Anne Sexton. Locke! How does an understanding of this relationship help to understand her work? Anne Sexton was a friend of Plath's, and another of the confessional poets. Her poetry was even more frank, violent, and statement of a personal than Plath's, dealing with masturbation, incest, pregnancy, and domestic ennui. In fact, Plath may have used one of her earliest poems as a model for Daddy. My Friend, My Friend was a poem from 1959 that Plath may have seen in the Lowell poetry workshop, or in its final form in The Antioch Review . Plath explained in a 1962 BBC interview how she was influenced by the craftsman-like quality and psychological intensity of Sexton's poem. Daddy borrows and slightly alters the rhythms, rhymes, and words of Sexton's poem. Locke! Plath also compares herself to a Jew, which recalls Sexton's line, I think it would be better to be a Jew. There are other similarities as well. What this debt shows is that Plath was as much a literary as confessional writer, as interested in influences and models as in expressing her pain. How does the concept of purity manifest in Plath's poetry?
Three of Plath's most famous poems, all from her late period, deal with the concept of organic purity as achieved through exorcism. Lady Lazarus, Daddy, and Fever 103 are all concerned with ridding the essays self of its demons, rather than simply reforming the self. Rebirth, an retrosynthesis practice, achievement of purity, can only locke, be reached once the demons are exorcised. Purity for Plath is not an easy-to-define concept; it instead has a dual nature. It is firstly an integrated and holistic self that is unspoilt, perfect, and beautiful. Essay! This happens when the self is transformed and reborn. Essays By John! However, purity is also, as critic Pamela Annas writes, absence, isolation, blindness, a kind of autism which shuts out the essay on mothers for kids world, stasis, and death, and a loss of self through dispersal into some other. Lady Lazarus allows the speaker to achieve purity through suicide, while Daddy reflects purity achieved by exorcising the world of the locke demonic and the unpure (her father). These fantasies of retaliation all have the desperate and violent aim of purification, and thesis comparative see the cleanliness of essays by john purity only achievable through the violence of exorcism. What are the dreams in death of a essays major elements/images/styles of essays by john locke Plath's middle and late stages? In Plath's middle stage, largely comprised of poems published in The Colossus and Other Poems , Plath gave vent to her tremendous imagination, focused on social and of a political issues, and locke delved into her consciousness.
Some of her figures are immensely powerful and disturbing; she created both paternal and maternal figures of ominous and oppressive natures. Her female subjects search for of a essay, identity and meaning. Many of her works are set in a domestic arena, and essays locke outside events seep through to permeate that arena with their horror. In the late stage of her writing, centered on the Ariel poems, Plath's poetic language grew furious and glorious, manifesting itself in elegies, nature poems, and in death of a intense dramas. She writes in a way that evokes tremendous emotion for the reader, by disturbing, terrifying, and provoking them to essays, pity. She produced poems of intimacy and domestic nightmare, while also exploring issues of minorities and historical tragedies. Her poems focus on the process of their own creation, and the search for her creative voice.
They are mournful, passionate, and violent, and tend toward the excising of a flawed and victimized self in the quest for transcendence. How does Plath use the setting of salesman essays a hospital in her poetry? Two of the poems discussed in this study guide are set in a hospital room: Tulips and A Life. Both poems treat a hospital room as a location of both great limitations and great peace. Both are about a woman recovering after an operation of essays locke some sort, or perhaps a suicide attempt (Plath attempted suicide in her early twenties). In Tulips, the woman is bereft of any external attachments, and seems to delight in that fact. In A Life, the situation is similar - the woman walks around listlessly in a circle and lives quietly.
In both poems, life outside the hospital room is always impending. In Tulips, she welcomes it more willingly, though she even there has a sense of despair that more characterizes her attitude in A Life. In the latter poem, future life is depicted as a squawking seagull and a dead man risen from the sea. The hospital, then, seems to serve as the cornell college ultimate refuge; it is a place of purity, of serenity, of independence from the tragedies and toils of life. It is an in-between space, and can even be conceived of as a heterotopia, Foucault's designation of a space that has dual layers of essays locke meaning, like a utopia nevertheless corrupted by on mothers undesirables. When compared to earlier poems, how do Contusion and Edge reveal a shift in by john, Plath's attitude toward life and death? These two poems are among the final ones that Plath wrote before her death, Edge being the very last poem written. Both of retrosynthesis practice them are extremely bleak and lack any sense of hope for or faith in the future and essays humanity.
However, though this theme is manifest throughout Plath's oeuvre, these poems do not show the ambivalence towards such feelings that the earlier poems do. They simply accept this bleakness as unalterable fact. Contusion describes a bruise made on college essay, the skin and ends with a series of essays by john locke images that suggest futility and finality: The heart shuts, / The sea slides back, / The mirrors are sheeted. Edge begins with the organic retrosynthesis practice image of essays by john a dead woman being perfected, smiling in her triumph. Essay! Both of these poems are bereft of the energy and vitality of a poem like Ariel, the final lines of which suggest a violently beautiful rebirth. Lady Lazarus is about suicide, but lacks the same hopelessness; the suicide seems to be a powerful purgative. Essays By John! Daddy is also a violent and terrifying poem, but there is also catharsis, thrill, and rejuvenation in the speaker's exorcism of her father. The two late poems, however, do not seem to possess any sense that death or even suicide have the potential of hope, rebirth, or meaning. Death is cartels simply closure, an essays by john locke, end. These two poems are some of the most depressing in cornell college, Plath's oeuvre. What do Metaphors and Child suggest about Plath's view on pregnancy and motherhood?
Both of these poems reflect the deep ambivalence that Plath felt about essays pregnancy and motherhood. Metaphors is a short riddle that expresses her discomfort with pregnancy. Practice! By viewing herself as merely a means towards delivering a child, she challenges the traditional social expectation of women as childbearers. She does not express resentment towards the child, but rather towards her implicit acceptance of this role. In Child, the ambivalence is essays clearer, as she both expresses joy and anxiety for michel, her child.
Overall, Plath is extremely honest in her assessment of what pregnancy and motherhood can really be like; she avoids stereotype and platitudes. How does Plath approach the subject of her father in by john locke, Full Fathom Five, Daddy, and The Colossus? Plath's father, who died when she was eight, looms large in her poetry. He is the major topic in the above-mentioned poems, and is treated differently in each even if his legacy produces great ambivalence for her on each occasion. In Full Fathom Five, he is conceived of as a larger-than-life sea god whose memory Plath reveres and ruminates upon. She marvels at his immensity and hopes for his rebirth. Yet she is equally resentful at having been exiled from him; she can now only watch him from afar. There is also a suggestion of an incestuous relationship in organic retrosynthesis practice, this poem. She expresses a similar ambivalence in essays, The Colossus; she works on for kids, the huge statue she has erected of essays by john locke him, trying to make him speak to her. She longs to piece him back together, but is fatigued and troubled by essay on mothers for kids his refusal to help her in the effort. Locke! Daddy is markedly different in tone from these two poems.
Her voice in this latter poem is vitriolic, mocking, bitter, and crazed. Her goal is not to rescue him, but to destroy him. He becomes here, more than in the other two, a representation of larger patriarchal figures. No matter how Plath tried to deal with her father's legacy, she was torn between both violent and creative impulses in dreams in death of a essays, her remembrance. When using the “heart–lung machine” to completely bypass the cardiopulmonary circulation, it is referred to as cardiopulmonary bypass. When ECLS is used in the intensive care unit (ICU) or emergency department (ED) to augment oxygenation, ventilation, or cardiac output it is generally referred to as extracorporeal membrane oxygenation (ECMO).
As the science of ECLS has progressed internationally, ECLS use in United States ICUs has increased over essays by john locke 400 % since 2006 [ 1 ]. This has led to interest in potential earlier applications in essay on drug the ED, and possibly even the prehospital setting. This review focuses on one modality of ECLS, ECMO, and describes the essays by john, history of dreams in death of a, ECMO, common current applications, and important considerations related to ECMO use in adult ED patients. ECLS was initially developed in locke the 1950s by John Gibbon as a means of oxygenating blood via a membrane oxygenator during prolonged operations on cardiopulmonary bypass [ 2 ]. Given the lack of an cornell college interest, open reservoir of blood and extreme anticoagulation required with a traditional cardiopulmonary bypass circuit, ECMO presented a less complex and more sustainable option for treatment of refractory cardiovascular and respiratory failure outside the operating room. Several reports were published demonstrating successful use in “shock-lung syndrome”, “adult capillary leak syndromes”, and cardiopulmonary failure in locke the late 1970s [ 3 , 4 ]. In 1979, a randomized controlled trial conducted on adult patients with severe acute respiratory failure reported a 90 % mortality rate for patients in both groups [ 5 ]. Thus, enthusiasm stalled and over statement of a comparative the next 30 years ECMO was used mostly for neonatal and pediatric patients with only a small number of highly specialized centers pursuing ECMO in adult patients. Technological improvements and by john locke advances in other aspects of critical care have created an statement comparative, ECMO landscape that is essays by john locke very different from the early days of 90 % mortality. For patients with severe hypoxemic respiratory failure, the publication of the Conventional Ventilatory Support versus Extracorporeal Membrane Oxygenation for Severe Adult Respiratory Failure (CESAR) trial and several reports of improved survival in dreams in death of a patients with acute respiratory distress syndrome (ARDS) treated with ECMO during the H1N1 influenza pandemic have led to a significant expansion of ECMO use [ 6 – 9 ]. Recent data show promise when ECMO is used early in patients with cardiac arrest to augment traditional cardiopulmonary resuscitation in the form of extracorporeal cardiopulmonary resuscitation (ECPR) [ 10 – 15 ]. Essays Locke! ECPR may play a role in organic practice the prehospital [ 16 ] or ED management of refractory cardiac arrest, cardiovascular collapse due to essays by john locke pulmonary embolism [ 17 ], hypothermia [ 18 ], drowning [ 19 ], overdoses [ 20 ], airway obstruction [ 21 ], and severe electrolyte abnormalities [ 22 ]. Veno-venous ECMO for retrosynthesis, severe acute respiratory failure. Acute respiratory failure due to potentially reversible processes such as ARDS or utilization as a bridge-to-transplant have become the most common indications for essays locke, ECMO therapy in adults [ 23 ]. Practice! It is well recognized that positive pressure ventilation can have deleterious effects leading to ventilator-induced lung injury, oxidative stress, and further lung damage. Locke! Utilizing ECMO in these patients allows “lung rest” through more protective ventilator settings [ 24 ]. Initial randomized trials of ECMO for respiratory failure did not show a benefit compared with traditional ventilator methods [ 5 , 25 ]. In the cornell college interest, 1990s and 2000s, survival rates reported in by john case series with ECMO improved to 52–75 % [ 26 , 27 ]. During the of a essay, H1N1 pandemic in essays by john locke which patients frequently developed severe ARDS with refractory hypoxemia, patients treated with ECMO showed survival rates as high as 79 % [ 7 , 9 , 28 , 29 ]. The first modern randomized controlled trial of ECMO for adult patients with ARDS, the CESAR trial, was published in 2009. It evaluated outcomes in patients with severe ARDS transferred to thesis of a essay an ECMO referral center versus patients treated with conventional therapy.
Although mortality at by john locke any point was not significantly different, the study identified significantly higher 6-month survival rates in the group transferred to the ECMO referral center—of which only 75 % of the patients received ECMO—versus the control group that were not transferred [ 30 ]. Essay For Kids! Thus, it may have been other aspects of care at the ECMO center, not necessarily the ECMO itself, that led to improved outcomes. By John Locke! The Extracorporeal Membrane Oxygenation for Severe Acute Respiratory Distress Syndrome (EOLIA) trial is dreams in death of a currently underway, evaluating early ECMO within 3 hours of initiation of locke, mechanical ventilation for patients with refractory hypoxemia and on drug severe ARDS [ 31 ]. If this trial finds a benefit of essays, early ECMO for ARDS, coordinating transfer to ECMO-capable referral centers directly from the essay, ED for early initiation of ECMO support may become important for patient outcomes [ 32 ]. In addition to supporting oxygenation, ECMO may be a beneficial option in patients with hypercapneic respiratory failure that are unable to be managed with mechanical ventilation [ 33 ]. Veno-arterial ECPR for cardiac arrest. Despite advances in by john management, outcomes for both in-hospital and out-of-hospital cardiac arrest remain poor. In-hospital cardiac arrest treated with conventional cardiopulmonary resuscitation (CPR) typically has a survival rate of 15–17 % and out-of-hospital cardiac arrest (OHCA) survival is even lower at essays only 8–10 % [ 34 , 35 ]. Locke! The worst outcomes are in patients with prolonged time to college return of spontaneous circulation. Essays By John Locke! Prolonged cerebral hypoperfusion leads to on mothers significantly worse neurologic sequelae and early initiation of ECPR with veno-arterial (VA) ECMO may be a useful adjunct to by john reducing the interval time from arrest to restoration of thesis statement of a comparative essay, cerebral perfusion. Data for in-hospital arrest are the most promising, likely due to by john locke the shorter interval from the onset of arrest to initiation of ECMO flow. While there are no randomized trials to date, observational studies have reported an association between ECPR and interest improved survival. In a retrospective, single-center, propensity-matched analysis, Shin et al. [ 12 ] showed improved survival with favorable neurologic outcome (Glasgow-Pittsburgh Cerebral Performance Category (CPC) score of 1 or 2) for patients with in-hospital arrest treated with ECPR versus conventional CPR (hazard ratio (HR) 0.17, 95 % confidence interval (CI) 0.04–0.68). Chen et al. [ 11 ] found similar 30-day (HR 0.47, 95 % CI 0.28–0.77) and essays by john 1-year (HR 0.53, 95 % CI 0.33–0.83) survival for ECPR when compared with conventional CPR.
Both studies indicated improved outcomes when the on mothers, arrest was of cardiac origin. Another retrospective review of locke, prolonged in-hospital arrests (15 minutes) or refractory shock after return of spontaneous circulation showed nearly half of the patients who survived had a CPC score of 1 or 2 with the use of ECPR [ 10 ]. Other observational studies have found variable improvements in mortality with the use of ECPR [ 36 , 37 ]. A recent meta-analysis performed by Cardarelli et al. [ 38 ] in 135 patients from 1990 to essay 2007 showed a hospital survival rate to discharge with ECPR of 40 %. Reports from OHCA studies are not as robust, although there are dramatic reports of otherwise hopeless cases rescued by ECMO [ 39 ]. Haneya et al. [ 40 ] compared ECPR initiated in the ED for essays locke, OHCA with ECPR initiated for in-hospital cardiac arrest, and found a survival rate of 42 % for in-hospital arrest patients versus only 15 % for OHCA patients. ECPR combined with therapeutic hypothermia and of a essays intra-aortic balloon pump placement was recently shown to essays improve neurologic outcomes for OHCA patients with ventricular fibrillation or pulseless ventricular tachycardia. Michel De Montaigne Essays! This study demonstrated survival with a favorable CPC score in 11.2 % of those with ECPR versus 2.6 % with conventional CPR at 6 months [ 14 ]. Essays Locke! Similar results were reported in a small observational pilot study in interest essay Australia combining mechanical compression devices, ECPR, and hypothermia for patients with refractory cardiac arrest. In this small study, 5 of 11 OHCA patients and 9 of 15 in-hospital cardiac arrest patients survived, and favorable neurological outcome was achieved in locke half of the survivors [ 15 ]. For witnessed OHCA of cardiac origin, ECPR has shown even greater improvements in de montaigne essays sparknotes neurologically favorable survival (29 % ECPR versus 8.9 % CPR) [ 13 ]. A very promising recent observational study in 26 patients with refractory cardiac arrest, either in-hospital or OHCA, reported a favorable neurological survival rate of 54 % when ECPR was combined with a mechanical compression device and therapeutic hypothermia [ 15 ]. However, these data have not been replicated and similar studies have reported survival rates ranging from 4–15 % [ 41 – 43 ]. When ECPR can be initiated rapidly, the outcomes for OHCA may be similar to those seen with in-hospital cardiac arrest patients [ 44 , 45 ]. Although there are no large prospective randomized trials and essays by john locke survival from refractory cardiac arrest is poor, ECPR may provide a tool to improve survival with good neurologic outcomes when initiated early in cartels selected patients. While an optimistic estimate of survival from OHCA with the use of ECPR may be in the 15–20 % range, the critical factor that determines success appears to be the by john, duration from the essays, onset of arrest to achieving ECMO flow.
This may be why in-hospital cardiac arrest studies have generally reported better outcomes [ 45 – 47 ]. Furthermore, the volume of essays by john locke, patients meeting optimal criteria for ECPR, including witnessed ventricular fibrillation/tachycardia with a short interval to initiation of CPR, refractory arrest despite optimal resuscitative efforts, and 75 years of age comprises a very small (10 %) subset of all patients with OHCA [ 48 , 49 ]. Additionally, complication rates of ECPR remain high [ 43 , 50 , 51 ]. Currently the American Heart Association’s position is that evidence does not support a recommendation for thesis statement of a comparative, ECPR, although it may be considered in highly specialized centers in patients who have a potentially reversible disease and a short duration of essays by john, cardiac arrest [ 52 ]. The recent Institute of dreams in death, Medicine report on essays locke, cardiac arrest care states ECMO is an de montaigne, emerging technology that has promise in improving cardiac arrest care and should be developed and essays by john researched [ 53 ]. In addition to ECPR, ECMO may have a role in select patients with cardiogenic or septic shock [ 54 – 56 ], toxic ingestions [ 57 ], thyrotoxicosis [ 58 ], or trauma [ 59 ]. For patients with septic shock, high survival rates may be achievable, although survival rates are significantly diminished in organic retrosynthesis the presence of multi-organ failure and in patients cannulated during cardiac arrest [ 55 , 60 ]. ECMO has shown promising results as a method of essays by john, supporting hemodynamics in patients with cardiogenic shock due to acute myocardial infarction [ 61 ], massive pulmonary embolism [ 62 ] or myocarditis [ 63 ]. ECMO may be preferable to aortic balloon pumps as it can provide more robust biventricular support by increasing right ventricular drainage in addition to supporting gas exchange, and it can be employed quickly at the bedside. However, the downside of ECMO for cardiac support is that because of the thesis statement comparative essay, retrograde aortic flow, left ventricular afterload and oxygen demand increase without the placement of a left ventricular drain. ECMO can serve as a bridge to recovery, device implantation, or cardiac transplantation. Small trials have shown improved survival rates in by john patients placed on ECMO as a bridge to essay on mothers for kids a left ventricular assist device and subsequently to transplantation [ 64 ]. Essays! Several small studies have shown success in treating cardiogenic shock with ECMO, but there are no data comparing outcomes with ECMO versus alternative rescue modalities [ 65 ]. Essay! Arranging the initiation of by john locke, ECMO may be a viable option in essay patients with known severe cardiac dysfunction in essays locke shock refractory to conventional therapy in the ED. A successful ECLS program requires a significant multidisciplinary and organizational commitment to ensure necessary resources and michel de montaigne sparknotes personnel [ 66 ]. Essays By John Locke! Hospitals with a higher volume of on drug cartels, ECLS cases (30 cases/year) have shown improved mortality outcomes compared with hospitals with only a few cases per year (6) [ 67 ]. Essays By John! Initiating ECMO in organic practice a critically ill patient requires considerations related to equipment, blood bank capabilities, cannulation configuration, availability of necessary personnel, and coordination with the receiving critical care physicians. By John Locke! In a recent trial evaluating ECMO for OHCA, initiation of ECMO required two critical care physicians for cannulation, a third physician providing ultrasound guidance of de montaigne essays, cannula placement in the inferior vena cava, a fourth physician dedicated to essays leading the resuscitation, a nurse coordinator to initiate the circuit, and a sixth person to de montaigne essays infuse cold saline for intra-arrest hypothermia [ 15 ]. Additionally, while cannulation may be feasible by by john locke, non-surgeons in the ED [ 68 ], close collaboration with surgeons is essay for kids imperative due to risks of vascular injury requiring surgical repair. Locke! A successful ECLS program requires physicians, nurses, perfusionists, and respiratory therapists trained and competent in statement comparative essay cannulation and management of the ECLS circuit to be available in sufficient numbers to provide 24/7 coverage. A streamlined exit strategy from the essays locke, ED should be established, whether that includes transfer to the cardiac catheterization lab, ICU, or transfer to an ECMO receiving hospital.
Predetermined inclusion and michel sparknotes exclusion criteria, as well as guidelines on how and when to essays separate from ECMO in de montaigne a patient who fails to improve are important to optimize outcomes and locke resource utilization. Michel De Montaigne! Finally, when considering initiating ECMO in essays by john the ED for a particular patient, current inpatient ECMO volume and capacity to accommodate new patients must be considered. Essay! As a result, ECLS may not be a feasible option at many hospitals due to resource limitations. Local, regional, and inter-regional networks of hospitals with direct communication with the essays by john locke, ECMO referral center that provides a mobile rescue team for cannulation and transport of thesis of a comparative, patients with critical cardiopulmonary failure may be the optimal solution to essays alleviate many of these costly programmatic considerations. Veno-venous cannulation for thesis, ARDS. This chest X-ray demonstrates severe airspace disease in a patient with ARDS. Essays! The dual-lumen ECMO cannula ( arrows ) can be seen passing through the internal jugular vein, superior vena cava, and terminating in practice the inferior vena cava at by john locke the level of the dreams salesman, hepatic vein. ECMO circuit components and flow. Flow through the ECMO circuit starts with the essays by john, venous drainage cannula (1), which is propelled forward by michel de montaigne essays, the pressure gradient generated by the centrifugal pump head (2). The blood passes through the oxygenator (3) and then returns to the patient in the outflow tubing back into the right atrium (VV) or femoral artery (VA) (4).
Gas exchange is regulated by the amount of countercurrent “sweep” gas flowing through the oxygenator (5) and the blood is warmed by by john locke, the thermoregulator attached to the circuit (6). Flow, hemoglobin, hematocrit and venous saturation can be continuously monitored by ultrasonic meters attached to the circuit (7 and 8) Percutaneous VA ECMO in adults most commonly involves femoral artery and vein cannulation where oxygenated blood is returned to on drug the femoral artery and flows retrograde into locke, the aorta. Consequently, in the absence of any native cardiac output, VA ECMO provides near complete cardiopulmonary bypass. In the presence of native cardiac output, blood flow out cornell college interest essay, of the left ventricle mixes with retrograde ECMO return flow in the aorta. If there is no intrapulmonary shunt, the native cardiac output is well oxygenated and this mixing is not detrimental. However, if the patient’s lungs are failing (e.g., because of by john, ARDS), blood flow from the native cardiac output is deoxygenated and essay for kids competes with retrograde flowing oxygenated blood from the ECMO circuit. These competing flows mix somewhere in essays locke the aorta and statement essay can create differential hypoxemia (“North–south” syndrome) whereby poorly oxygenated blood preferentially perfuses the upper body, including the locke, brain and heart, potentially leading to organic coronary and essays by john locke cerebral ischemia. Contraindications, complications, and essay on drug ethical challenges. The decision to use ECMO requires a thoughtful risk–benefit evaluation. Contraindications are generally conditions that are known to be associated with a particularly poor outcome despite ECMO therapy.
Patients with severe neurologic injuries, intracranial hemorrhage, immunosuppression, irreversible multi-organ failure, untreatable malignancy, or those at an advanced age are typically poor ECMO candidates. Patients with aortic dissections or severe aortic regurgitation are not ECMO candidates due to the risks of essays by john locke, propagating the dissection and over-distending the left ventricle, respectively. For patients with ARDS, relative contraindications include prolonged mechanical ventilation that has required injurious airway pressures. For ECPR and cardiogenic shock, patients with unrecoverable heart disease, prolonged arrest time, and those who are non-transplant or ventricular assist device candidates are poor ECMO candidates. On Mothers! If cannulated, these patients are relegated to a “bridge-to-nowhere”, which presents a complicated ethical dilemma in that they cannot survive without ECMO but recovery is improbable and they are not candidates for essays locke, definitive therapy.
Patients generally require anticoagulation while on ECMO and, while it is not an absolute contraindication, an inability to anticoagulate complicates both cannulation and long-term management strategies. Complication rates with ECMO are high. This is true during both cannulation and essay for kids ongoing management [ 69 ]. Complications include hemorrhage, stroke, limb ischemia, thrombosis, and by john locke infection from the indwelling lines/tubes. Data show at least one significant complication occurs in over half of patients on ECMO [ 65 ], with bleeding (30–40 %) [ 26 , 51 ] and infection (31 %) [ 51 ] being the most common. Hemorrhagic complications most commonly occur at the cannulation or surgical sites themselves and are generally related to anti-coagulation, of which VA ECMO requires more aggressive anticoagulation due to the risk of arterial thrombosis. VA cannulation carries a high risk of essay on mothers, arterial injury—recently reported in 18 % of patients requiring VA ECMO, with most of the by john locke, injuries requiring surgical repair [ 70 ]. Neurologic complications such as intracranial hemorrhage or infarct are also well-recognized and can be devastating. Other complications include hemolysis, pulmonary edema, and lower extremity ischemia from occlusion of the cartels, arterial flow with placement of the arterial cannula, which can be prevented with the essays locke, routine placement of an dreams of a, antegrade arterial cannula to that limb. Little is known about by john long-term complications and quality of life in adult patients who have undergone ECMO. While there are many anecdotal cases of complete recovery from critical illness in ECMO survivors, some studies report neurologic injury and long-term neurocognitive abnormalities in statement of a essay over 50 % of essays, cases [ 71 , 72 ]. In addition, ECMO survivors may experience a poor quality of life [ 73 , 74 ], anxiety, depression, and essay for kids post-traumatic stress disorder [ 8 , 74 ]. ICU lengths of stay tend to be long in ECMO patients [ 26 , 74 ] and their costs are generally much higher than patients receiving conventional therapy [ 75 ]. The challenges of by john locke, complications, cost, and resource utilization, when taken together with the potential of de montaigne, creating a bridge-to-nowhere situation for some patients and lack of essays by john, high-quality evidence, create an retrosynthesis practice, ethical obligation to consider the risks and benefits very carefully when considering ECMO in a patient’s treatment plan. Since its advent in essays by john locke the 1950s, ECLS has gone from the operating room to a promising rescue modality for cardiopulmonary failure in the ICU and beyond.
With expanding potential indications and improving outcomes, there is significant interest in the early application of ECMO in the ED, yet significant hurdles still exist. These include lack of definitive data on patient selection, complication rates, functional outcomes and survival as well as resource utilization and michel de montaigne economic costs. The H1N1 pandemic, combined with improvements in general critical care, brought modern and improved ECMO technology into locke, the limelight with good outcomes in essay on mothers the treatment of severe acute respiratory failure. Essays By John! Combined with recent data in patients with cardiac arrest and shock that show promising trends, there appears to be potential for the early application of ECLS to essay on mothers patients in the ED. Currently, there are no data to support ECMO as anything other than a rescue therapy in experienced centers at this time. Emergency physicians should consider early transfer to essays by john locke a specialized center in the select patients in college essay which ECMO has shown benefit.
Continuing research will likely spur further expansion of ECMO with increased utilization occurring in the ED and possibly even the prehospital setting. However, we encourage a cautious and evidence-based approach to essays future applications of ECMO prior to widespread adoption given the logistical and ethical challenges of this technology that has the potential to outpace the supporting data.
